Donation Cards and Tags should be filled out and included with every donation you make.

Instructions:
Each box you donate should have one of the donation cards and each item should have a donation tag. The card can be placed inside the box on top of your donations. The tags can be attached to your items by a simple piece of yarn. You may choose to write "In memory of ...." on the back of the tags if there's a baby or babies you are doing this in memory of. Please do not use staples or paperclips. Thank you!
